Put an elbow on it first. To everyone saying there has to be another one, no there doesn’t have to be. Newer builds there most certainly is but an older home this may be it.

yep this is a sewer cleanout – got two of those on older homes. You can add an adapter yo add an elbow.

make sure nobody was on the toilet for at least a couple of minutes and make sure no water is running in the house.

Then unscrew very slow 😉 when lots of water is coming through the threads – think twice.

get a macerator, stick the hose down the pipe some… I bet it drops down at some point.
